SERVICE AGREEMENT: I understand that by signing this Service Agreement & Security Deposit Form I have entered into an Agreement to secure services with and Agree To Pay for services rendered through ADEK Productions, 1801 West Street, Suite 105, Annapolis, Maryland 21401, whether secured in person, over the phone, or online. I have read and agree to the online Services Policy.
DEPOSIT/PAYMENT TOWARD INVOICE: By signing this Service Agreement, I agree to pay for services invoiced and that my digital signature, represents my authorization for payment toward invoiced services. This card will be used for your Hold Authorization/Security Deposit - required for all rentals and services.
HOLD AUTHORIZATION/SECURITY DEPOSIT: (Applies to rentals, live services, and studio related services only.)
By signing this Service Agreement, I agree that my digital signature, represents my authorization for payment toward invoiced services and the required minimum Security Deposit amount as indicated on my invoice. I understand that the Security Deposit is a "hold authorization" on my credit card held to cover additional services and/or fees (extended period fees, add-on items, late fees, repair or replacement fees, etc.) if applicable, related to services and rentals. In the event that the hold authorization amount does not cover all additional expenses, I understand that I am responsible for balances owing. If not applicable, the Security Deposit held will be released in approx. 7-10 days, automatically. It may be released sooner and at times may take longer.
DIGITAL SIGNATURE: I understand that as a convenience, my digital signature, as submitted, will serve as and be used in lieu of an in-person hand written signature on credit card receipts for transactions conducted with ADEK Productions.
ID VERIFICATION: We may require that you present photo ID for verification upon payment.
